Pain Management

We understand that pain can massively disturb one’s functioning and can hinder the progress of healing. We help our patients to be as comfortable as possible to participate in and benefit from therapy.

Orthopedic Rehab

Allow our experienced rehabilitation team to develop a custom orthopedic therapy plan for you so you can begin to restore your range of motion and mobility.

IV Therapy

Our Registered Nurses are available 24/7 to administer IV Therapy, to help you regain independence and spend less time in the hospital

Wound Management

We have a Wound Specialist MD that performs weekly rounds. Our visiting physician and certified Treatment Nurse care for wounds gently and respectfully, developing a custom treatment plan fit for each individual in their unique stage and promote wound healing and prevent a recurrence.

Nutrition

Our Registered Dietitian and Kitchen staff offer a choice of healthy, nutritious and tasty meals that work with each patients dietary requirements.

Social services

Social Services primary role is to advocate for each resident on their road to recovery. Our Social Services team is here to ease the transition upon admission all the way through discharge. We want to ensure that each patient is comfortable during their stay and is as prepared as possible to discharge home.

We have grown a vibrant and uplifting atmosphere to set the tone for each individual’s stay. Our staff is very supportive, friendly and companionable. We have developed social gathering programs and activities to offer recreational flavors to our residents.

COVID-19 Updates

Santa Cruz Post Acute (SCPA) is making every effort to prevent the exposure of COVID-19 to our residents and staff. Our number one priority is the safety of all the individuals who live in our community. We closely follow the directives from CMS, California Department of Public Health (CDPH), and Santa Cruz Communicable Disease Division. Visitation is allowed – All visitors must check in with the front desk upon arrival for temperature check and further direction for a safe visit. We had a COVID exposure reported on 10/5/22 and have since had a few residents test positive – symptoms have been mild to non-existent and treatments are offered by our physicians. SCPA adheres to CDPH guidance in AFL 22-13.1 for infection control measures. Reception can provide more information upon entering and completing a COVID screening.

COVID-19 Vaccines

All team members without medical or religious exemptions are required to be fully vaccinated for COVID-19.

Our staff practice social distancing guidelines wherever possible.

PPE (Personal Protective Equipment) in use

PPE including surgical masks, gloves, gowns and N95 masks are used in line with infection control guidelines.

We've increase our cleaning and disinfecting schedules helping to keep everyone safe.

In person visits

Keeping you connected to your loved ones is important and our visitation guidelines meet local, state and federal regulations.

Hygiene and handwashing

Our staff follow strict handwashing protocols and encourage our patients to do the same.
