Jessica was super helpful in supporting our family with transitioning my family members’ care from a facility where he was located hours away, making it difficult for us to visit and follow up with in person care. Thank you Jessica for giving our family an opportunity to support our uncle from a closer distance. You’re hard work does not go unnoticed. 😊

So happy with the care at santa cruz pist accute..Jessica was absolutely wonderful placing my mom in a great room next to a window facing the birds 🐦 with made my mom’s stay there so pleasant..the staff was friendly and efficient the facility always clean the level of care was TOP NOTCH ..so very grateful 🙏 that my mom was in such great care.

My brother is only 50 yrs old and he had a tragic medical incident that caused him to be paralyzed from he’s waste down. I have been trying to transfer him from Fresno back home and it has been a challenge. No facility wanted to take him because of he’s age and other factors and we are so blessed that after working with Jessica this may become a reality. I really appreciate her patience of dealing with my calls and really working with me and having the heart for my family and brother. I can’t thank her enough Jessica is an Angel that God has put in my path.

I broke my hip and femur and was blessed to stay and heal and Santa Cruz Post Acute. The Staff was friendly, patient,and understanding. Above all else the staff was optimistic and kept a sense of humor with me . Laughter is the best medicine. I been through a couple other rehabilitation skilled nursing facilities where my mom has been a resident but nothing so far compares to the professionalism & welcoming and supportive staff that has need provided here at this facility since day 1. I’m very pleased with their leadership and dedication to take care of my mom’s needs and helping me and my siblings with preparing and filling documentation needed. We’ve had an excellent experience with Santa Cruz Post Acute Care. . My dear friend has been a resident and the level of care she’s receiving has been outstanding. The nursing and physical therapy staff are incredibly attentive and always updates us promptly on any medical changes. Additionally, the Management staff is thoughtful, professional and highly responsive. .They consistently take the time to ensure that every resident receives the highest quality of care. The facility itself is always clean, fresh, and welcoming. We are incredibly grateful for the peace of mind this community provides.

My husband was recently at Santa Cruz Post Acute following surgery. The staff was attentive, competent and caring. We were both grateful for the care he received there.

When we found out that my father-in-law would have to go to a skilled nursing facilty for an extended amount of time, we were really nervous as many local facilities don’t have the best reputation. Santa Cruz Post Acute was recommended by the hospitalist and so we decided to have him take residency there for a few months. We have been very happy with our choice – SCPA has exceeded all expectations! The facility is newly renovated and is light and clean and the layout is very well thought out. The care team and nurses have all been very kind and my father-in-law (who can be difficult/grumpy) actually seems to have built a good rapport with some of his “favorites” (which is unheard of). A few days after he was admitted, we had a case management meeting and we were pleasantly surprised to find out how much the SCPA staff does beyond taking care of his medical needs. Mayra (his case manager) is helping us think through what needs to happen to line up care after he is released and is helping us with his MediCAL application. Sandy (his social worker) is helping with arranging transport to doctor appointments and has been very helpful with all other odds and ends. The nutrionist and also the head nurse Anne were both so kind and funny – we walked away from that meeting feeling much lighter and less confused about how this all works. Working in a skilled nursing facility is not an easy job but it seems that everybody who we have interacted with genuinely enjoys being there and is taking pride in what they do. Thank you for making this difficult time for our family a more positive experience!

Santa Cruz post acute took wonderful care of my husband while he was there. Staff was always willing to help him and me with any questions. CNA’s were amazing with him and his therapy workers were such a gift. Their patience with him was greatly appreciated. Thank you to everyone again

Great, welcoming facility for nursing students as well as residents/patients. Facility well maintained, bright, clean. Staff knowledgeable and caring.

I’m writing this review because Santa Cruz Post Acute has been good for my father. He is rehabbing post stroke and is progressing with his therapists. The staff has been very helpful in making sure they are attentive. I’m happy that Santa Cruz Post Acute is trying to do a good job. Not all nursing facilities are bad.

My loved one passed away at Santa Cruz Post Acute, and our experience there was heartbreaking. She was transferred from a Monterey hospital after femur surgery to receive physical therapy and become strong enough to return home, but instead her condition declined under their care. During her stay, she lost a considerable amount of weight, and although her surgical wound had healed, she developed bed sores on the bottoms of her feet and lower back that were not properly communicated to the family. We witnessed isolation, rough handling, poor hygiene, poor communication, and lack of compassion. The food was poor, and staff often took a very long time to respond, sometimes hours. She was also frequently in a very deep sleep during the day, which raised concerns about possible overmedication. Her clothes went missing, staff would turn off her cell phone, and with her TV not working, she was left even more isolated. Vulnerable patients deserve dignity, attentive care, hygiene, and transparency with family. That was not our experience. Sally M

I recently had to help my senior Aunt & Uncle find a good rehab facility in the Santa Cruz area after a hospitalization. I toured several & upon a warm welcoming greeting at Santa Cruz Post Acute by the director, Jessica Moran, & Admission Coordinator, Mayra Serrato I began my tour of the facility. I was instantly impressed with the energy, lightness, organization & cleanliness of the facility . The staff was busy with client activities which also impressed me with the energy & management. It was obvious that the interaction I saw constantly between nurses, care staff & residents was key to the comfortable atmosphere that was present throughout the facility. My Aunt & Uncle have been patients there for several weeks & the high grade of care, physical therapy & professional, friendly treatment is comforting for me. The Santa Cruz Post Acute provides a level of care & management that should be recognized as an example for other Assisted Living facilities. Santa Cruz Post Acute saved my brothers life. They have been wonderful. He came to them with broken back, ribs and was being evaluated for hospice care. He had very poor care at another facility where he had been for a year+ after a stroke. He had declined there and was unable to move, sit up, speak or eat when I insisted that he go back to Dominican Hospital. They said he had no broken bones and was “failing to thrive and should be in hospice care” He was dying…. The hospital staff was amazing. They confirmed that he indeed had broken bones from a fall at facility, got him stabilized and facilitated a move to a different facility. Within a few weeks with the wonderful care givers at SCPA, he was eating, speaking, and working on sitting up. Finally out of pain and much happier :). I just returned from a visit today, after just a couple months, he’s sitting up on his own, is speaking better, can walk with assistance, and has mastered some basic skills. From the day he was admitted every staff member has been great. There was more communication and followup in the 1st week than the entire time he was at the previous SNF (over a year). I live out of state. What a difference. Thank you all for saving my brother’s life. Can’t wait to see the progress he’s bound to make over the next few weeks. I’m sleeping better knowing that he’s in great hands.

I am writing to express my sincere gratitude for the exceptional care my mother has received at Santa Cruz Post Acute since her transition from the hospital in February. Jessica has been absolutely amazing from day one. Navigating skilled nursing was a new and challenging process for our family following my mother’s stroke, especially while coordinating care from out of state. Jessica and her team have been incredibly patient and kind, guiding us through the process and effectively facilitating communication with both the care team and insurance. Her support significantly eased my anxiety during this transition. I also want to commend the rehabilitation team. We have seen tremendous progress in my mother’s recovery, and we are grateful for the hard work and dedication they show their patients. Additionally, I cannot say enough about the wonderful nursing and CNA staff, specifically Christian, Miriam, Arellia, and Kelly. It is clear that they truly love what they do, and the quality of care they provide to my mother is outstanding.
