Great, welcoming facility for nursing students as well as residents/patients. Facility well maintained, bright, clean. Staff knowledgeable and caring.
I’m writing this review because Santa Cruz Post Acute has been good for my father. He is rehabbing post stroke and is progressing with his therapists. The staff has been very helpful in making sure they are attentive. I’m happy that Santa Cruz Post Acute is trying to do a good job. Not all nursing facilities are bad.
My loved one passed away at Santa Cruz Post Acute, and our experience there was heartbreaking. She was transferred from a Monterey hospital after femur surgery to receive physical therapy and become strong enough to return home, but instead her condition declined under their care. During her stay, she lost a considerable amount of weight, and although her surgical wound had healed, she developed bed sores on the bottoms of her feet and lower back that were not properly communicated to the family. We witnessed isolation, rough handling, poor hygiene, poor communication, and lack of compassion. The food was poor, and staff often took a very long time to respond, sometimes hours. She was also frequently in a very deep sleep during the day, which raised concerns about possible overmedication. Her clothes went missing, staff would turn off her cell phone, and with her TV not working, she was left even more isolated. Vulnerable patients deserve dignity, attentive care, hygiene, and transparency with family. That was not our experience. Sally M

I recently had to help my senior Aunt & Uncle find a good rehab facility in the Santa Cruz area after a hospitalization. I toured several & upon a warm welcoming greeting at Santa Cruz Post Acute by the director, Jessica Moran, & Admission Coordinator, Mayra Serrato I began my tour of the facility. I was instantly impressed with the energy, lightness, organization & cleanliness of the facility . The staff was busy with client activities which also impressed me with the energy & management. It was obvious that the interaction I saw constantly between nurses, care staff & residents was key to the comfortable atmosphere that was present throughout the facility. My Aunt & Uncle have been patients there for several weeks & the high grade of care, physical therapy & professional, friendly treatment is comforting for me. The Santa Cruz Post Acute provides a level of care & management that should be recognized as an example for other Assisted Living facilities. I want to express my appreciation with SCPA’s Business Development Director, Jessica Moran, who has been very warm and welcoming in assisting us with transitioning my disabled sister to SCPA. Jessica and staff have been attentive in getting my sister settled into her new environment with their care giving duties to asure she is comfortable and giving us peace of mind she is being well taken care of. Thank you Jessica and all the staff at SPCA to your dedication and caregiving duties!

Santa Cruz Post Acute saved my brothers life. They have been wonderful. He came to them with broken back, ribs and was being evaluated for hospice care. He had very poor care at another facility where he had been for a year+ after a stroke. He had declined there and was unable to move, sit up, speak or eat when I insisted that he go back to Dominican Hospital. They said he had no broken bones and was “failing to thrive and should be in hospice care” He was dying…. The hospital staff was amazing. They confirmed that he indeed had broken bones from a fall at facility, got him stabilized and facilitated a move to a different facility. Within a few weeks with the wonderful care givers at SCPA, he was eating, speaking, and working on sitting up. Finally out of pain and much happier :). I just returned from a visit today, after just a couple months, he’s sitting up on his own, is speaking better, can walk with assistance, and has mastered some basic skills. From the day he was admitted every staff member has been great. There was more communication and followup in the 1st week than the entire time he was at the previous SNF (over a year). I live out of state. What a difference. Thank you all for saving my brother’s life. Can’t wait to see the progress he’s bound to make over the next few weeks. I’m sleeping better knowing that he’s in great hands.

I am writing to express my sincere gratitude for the exceptional care my mother has received at Santa Cruz Post Acute since her transition from the hospital in February. Jessica has been absolutely amazing from day one. Navigating skilled nursing was a new and challenging process for our family following my mother’s stroke, especially while coordinating care from out of state. Jessica and her team have been incredibly patient and kind, guiding us through the process and effectively facilitating communication with both the care team and insurance. Her support significantly eased my anxiety during this transition. I also want to commend the rehabilitation team. We have seen tremendous progress in my mother’s recovery, and we are grateful for the hard work and dedication they show their patients. Additionally, I cannot say enough about the wonderful nursing and CNA staff, specifically Christian, Miriam, Arellia, and Kelly. It is clear that they truly love what they do, and the quality of care they provide to my mother is outstanding. My mom stayed there after her surgery. The staff were very friendly provided loving care. Jessica, Tina, George and Fraddie are the best coordinating my mom’s doctor visit and discharge time for supportive equipment. Thank you all!

The facility itself was clean, well-maintained, and had a calm, reassuring atmosphere. I especially appreciated how the staff took the time to treat residents with dignity and respect, which made a big difference in my overall experience. Overall love the community.

I found the staff at Santa Cruz Post Acute very caring. The CNA truly cared about her patients and made them comfortable and happier. The therapists were excellent, dedicated and brought joy and encouragement to their work with patients and help them immensely. Care conferences with the case managers and team were regular and helpful in optimizing care. I also found the administrators responsive and helpful with any needs that came up. I would definitely recommend this facility.

We think S C Post Acute Care’s Director of Business Development is an outstanding model of helping us and other relatives or friends. I am grateful for her taking us under her guidance. We arrived having flown in from central México confused about my sister’s diagnosis, prognosis and future plans. Jessica gave us all the necessary medical information and advice. She was always available to assist us with our 10 day visit. We felt confident that my sister was in an excellent place. When we arrived back home, I called her asking she go to my sister’s room, hug her and assure my sister we loved her. Jessica didn’t hesitate to do just that. My mom was very well cared for at Post Acute. The nurses on staff are friendly, caring, and compassionate, and were there for her when she needed assistance. The admin staff was responsive when we had questions, including the receptionist, Tina, who is friendly and very always there to help. Space is somewhat limited in the shared rooms, but nurses and staff kept things as orderly as possible.the facility is clean and there’s a sunny courtyard as well as a large main room, both good places for use while visiting. All in all we were happy with Santa Cruz Post Acute.

From the moment I walked in, the staff made me feel genuinely welcomed. This was my father’s first skilled nursing facility, and naturally it was a big transition for our family. When I asked if he could be moved to a different room, the staff didn’t hesitate — they listened, understood, and made the accommodation right away. That kindness meant more than they probably realize. Throughout this stage of my father’s life, the staff has been consistently caring, thoughtful, and respectful — not just toward him, but toward our family as well. Knowing he is in a place where people truly care has brought us comfort and peace of mind. I am very grateful for the compassion and professionalism shown by everyone here. I will always be grateful for the kindness shown to my father and to us.

My husband is a 79 year old Viet Nam vet who has been at the SCPA for the 20 days covered by Medicare after a hospital stay. He has been given the great and caring care by the staff. He has many medical issues plus has Alzheimer’s..He is a huge fall risk which adds to the difficulty in his care. I wish he could stay there, but it is not a contracted facility with the VA. He has to be moved and I am extremely sorry for that. There is truly not enough places for a vet to go.I cannot say enough about SCPA. Diane Dearinger

Very Poor quality health care. Health and safety violations. I had to call 911 because no RN or on call Dr could be found or contacted, I was tachycardic and high blood pressure. The CNA Looked for help for approximately 15 minutes, my heart rate was climbing. So I called 911 I was served food that was listed as known allergens. Served under cooked pork and chicken. Given the wrong medication and then not given 1 important medication I needed. Never took my glucose testing early morning or timely, usually 30 minutes or longer after I had eaten. My roommates were left in soiled under garments sometimes over an hour. Unhealthy bed pads. I bought my own bed pads and undergarments. CNA took approximately 20 minutes or longer sometimes to respond. The physical therapy and occupational therapy did a good job! This place should be shut down. All in all it is Unsafe and Unhealthy.
