The facility itself was clean, well-maintained, and had a calm, reassuring atmosphere. I especially appreciated how the staff took the time to treat residents with dignity and respect, which made a big difference in my overall experience. Overall love the community.

I found the staff at Santa Cruz Post Acute very caring. The CNA truly cared about her patients and made them comfortable and happier. The therapists were excellent, dedicated and brought joy and encouragement to their work with patients and help them immensely. Care conferences with the case managers and team were regular and helpful in optimizing care. I also found the administrators responsive and helpful with any needs that came up. I would definitely recommend this facility.

We think S C Post Acute Care’s Director of Business Development is an outstanding model of helping us and other relatives or friends. I am grateful for her taking us under her guidance. We arrived having flown in from central México confused about my sister’s diagnosis, prognosis and future plans. Jessica gave us all the necessary medical information and advice. She was always available to assist us with our 10 day visit. We felt confident that my sister was in an excellent place. When we arrived back home, I called her asking she go to my sister’s room, hug her and assure my sister we loved her. Jessica didn’t hesitate to do just that. That action made my heart and day so much better. Thank you Santa Cruz Post Acute Care’s for employing Jessica Moran, an outstanding person. Sally and Mike Munde.

My mom was very well cared for at Post Acute. The nurses on staff are friendly, caring, and compassionate, and were there for her when she needed assistance. The admin staff was responsive when we had questions, including the receptionist, Tina, who is friendly and very always there to help. Space is somewhat limited in the shared rooms, but nurses and staff kept things as orderly as possible.the facility is clean and there’s a sunny courtyard as well as a large main room, both good places for use while visiting. All in all we were happy with Santa Cruz Post Acute.

From the moment I walked in, the staff made me feel genuinely welcomed. This was my father’s first skilled nursing facility, and naturally it was a big transition for our family. When I asked if he could be moved to a different room, the staff didn’t hesitate — they listened, understood, and made the accommodation right away. That kindness meant more than they probably realize. Throughout this stage of my father’s life, the staff has been consistently caring, thoughtful, and respectful — not just toward him, but toward our family as well. Knowing he is in a place where people truly care has brought us comfort and peace of mind. I am very grateful for the compassion and professionalism shown by everyone here. I will always be grateful for the kindness shown to my father and to us.

My husband is a 79 year old Viet Nam vet who has been at the SCPA for the 20 days covered by Medicare after a hospital stay. He has been given the great and caring care by the staff. He has many medical issues plus has Alzheimer’s..He is a huge fall risk which adds to the difficulty in his care. I wish he could stay there, but it is not a contracted facility with the VA. He has to be moved and I am extremely sorry for that. There is truly not enough places for a vet to go.I cannot say enough about SCPA. Diane Dearinger

Very Poor quality health care. Health and safety violations. I had to call 911 because no RN or on call Dr could be found or contacted, I was tachycardic and high blood pressure. The CNA Looked for help for approximately 15 minutes, my heart rate was climbing. So I called 911 I was served food that was listed as known allergens. Served under cooked pork and chicken. Given the wrong medication and then not given 1 important medication I needed. Never took my glucose testing early morning or timely, usually 30 minutes or longer after I had eaten. My roommates were left in soiled under garments sometimes over an hour. Unhealthy bed pads. I bought my own bed pads and undergarments. CNA took approximately 20 minutes or longer sometimes to respond. The physical therapy and occupational therapy did a good job! This place should be shut down. All in all it is Unsafe and Unhealthy.

The facility is kept very clean, majority of staff kind and compassionate. Having spent time in many Skilled care homes over the years with elderly parents, family in rehab, etc. I do find this to be very loud. I have never been somewhere where the staff holler back and forth down the hall, out from the rooms, cnas laughing and conversing outside of patient doors. Mobile patients rolling around playing music even. I also feel that the glass sliders in rooms are nice, but…one can see right in. Perhaps need to be tinted. A man’s room is right across the patio from my sister’s room. Often staff doesn’t pull blind while changing her. Those are things I’d keep an eye on if you’re finding yourself or a loved one staying at Santa Cruz Post Acute.

I highly recommend this nursing facility. I sent my father there because of our doctor’s recommendation. I cannot tell you what a peace of mind, and help they have been with every facet of my father’s stay. Especially Jessica. She was a Godsend

I highly recommend Santa Cruz Post Acute for your love ones. I visited several other facilities prior to registering my mother. It is managed by considering every individual patient care and need in mind. I can’t thank Jessica Moran, Business Director, enough for providing such high level of care.

They do not answer the telephone even during normal business hours. Today, October 28, I have called four different times and have hung up after being on hold for 20 minutes each time. It’s only 3:40 PM and they have not answered their phone in 2 1/2 hours. I’ve left two messages and never received a call back. The patient that I am Involved with is important to me and I need to reach this medical facility and they don’t answer the phone and that is not a good sign. ** Update to my previous review.** I have called Santa Cruz Post Acute Facility and left five messages over the past three days during normal business hours and after business hours and I have yet to receive a return call. I just called again at 3:15 PM which to me is in the middle of the business day and still, the phones are going unanswered. I don’t understand how I can leave multiple messages over multiple days and not one single person has responded. It makes no sense and it makes me start doubting the overall care at this facility and I do not want to question that. I’ve heard very good things, but unfortunately, I’m not experiencing any of them.

I spent nearly a week here after breaking my tibia in a motorcycle accident. I was brought from Stanford University Hospital late at night and was put in a bed immediately. In the morning, I awoke to no one greeting me, or checking in on me. The windows I looked out at were dirty and the screens had cobwebs on them. (I told them the way they care for their windows says a lot about how they share their patients). I awoke in the morning and felt alone and disoriented. Finally, I rang for a nurse and shared my concerns and observations. She jumped on it and later that afternoon, I had four managers at the foot of my bed listening to my concerns, writing them down and asking me questions. The next thing I know, my windows and screens are being cleaned, the physical therapist stops by and asks what I need, the nutritionist comes in and asks me my meal preferences. My nurses begin to check on me more often. Everyone was doing their best to make sure my stay was professional and impactful. I gave them four stars because they responded quickly to my advocacy of myself, that I also think impacted others there as well. I saw the windows of other rooms getting cleaned. Some staff who stood out: Belinda, an excellent CNA nurse, Kelly, medications nurse, Jessica, business development, Dan, Eric & Nancy of the PT area, Tyler, Alejandra, Joy (a medication nurse), I felt fully valued by these people. I do recommend this facility, just know, if you have concerns, voice them immediately. They will listen and act on them. I checked out after they diligently confirmed a gurney transporter to Stanford Hospital for my final surgery. Thanks to everyone there who rose to my concerns and went out of their way to fulfill my concerns that made me feel comfortable. Make sure you advocate for your or your family member’s concerns!

My mother Ruth has been in the facility since March and it has been a breath of fresh air for her and myself. She is a long term patient and has made so many huge steps since shes been there. Number 1, physical therapy is amazing my mother had been bed ridden for 2 years and since she’s been in the facility she is able to stand, walk with help, gets around in a wheel chair. It’s mind blowing to see the level of care that they have given her. I truly am grateful for all the communication that is given to me. The CNAs love my mother and are always happy to see her. Adriana who is a CNA there is absolutely amazing and is so loving to my mother thank you so much. Bryan the nurse is great and is always good to my mother. There are so many to name. Thank you again for all the care you give to my mother. Santa Cruz nursing facility has been amazing thank you again.

Our family member came to Santa Cruz post acute with a very complicated diagnosis and recovery plan. The facility stepped up admirably to not just meet his needs, but to consistently treat him like an important guest, rather than a difficult case. Before his stay here, he had been at several other skilled nursing facilities outside of Santa Cruz. Santa Cruz post acute has been, hands-down, the best. In particular, Matthew provides a level of attention to detail that has exceeded any other infectious disease nurse we have met. The entire facility, however, seems to be well run and well-staffed with people who care. Although occasional mistakes will be made in every facility environment, the staff has been quick to acknowledge any errors and even quicker to fix them. This kind of work is very hard to do, and so important. We highly recommend Santa Cruz post acute for you and your loved-ones.

Santa Cruz Post Acute nursing home is a clean, bright, and welcoming place. From the moment I arrived, the staff gave me their full attention and the administrator kindly showed me around. The admitting staff were kind, polite, and highly professional. I noticed that the residents looked comfortable and well cared for, which was very reassuring. The admission process was quick and I appreciated how each case seemed to be handled with genuine care according to individual needs.
